Purchase a List (Roster) of Licensees
The ND Board of Chiropractic Examiners has a roster/mailing list of all licensed Chiropractors (D.C.'s) and Certified Chiropractic Clinical Assistants (CCCA's) available for purchase. The fee of $100.00 includes the roster information for all D.C.'s and CCCA's. You may choose to receive the information for one group only, but the fee does not change.
To purchase a list or roster of all license holders, please:
- download and complete this form;
- include a check or money order, in the amount of $100.00, payable to the ND Board of Chiropractic Examiners (we are not able to accept credit cards for this request);
- include the email address you want the list submitted to in your request.
The information provided on the roster will include the names, mailing addresses, license numbers, license status, issue dates, expiration dates, and any disciplinary action (if applicable). We do not provide social security numbers, birthdates, phone numbers, or email addresses.
