Request License Verification of your ND License or Certification
The North Dakota Board of Chiropractic Examiners will complete official license verification requests, sometimes referred to as a letter of good standing, for all current or previously licensed individuals.
There is no fee for this service.
To request a license verification, please contact the board office, preferably via email, to request. Please include any special instructions in your request, such as:
- Mailing address for the verification.
- Where is the license verification letter to be sent?
- Email address.
- Can the request be submitted electronically?
- If yes, what email address do we use?
- Designated form (if applicable).
- Does the jurisdiction require a specific form to be completed?
- License Holder Contact information
- Your contact information if we have any questions regarding the request.
