Requesting a List (Roster) of Licensees
To request a list or roster of all license holders, please submit the Mailing List Request Form.
There is no fee for an electronic list.
Appropriate copying chargers apply if requesting a paper list ($0.25 per page).
The information provided will include:
- Names of License Holders
- Business Mailing Addresses
- Email Addresses
- License Numbers
- License Issue Dates
- License Expiration Dates
- License Status
Personal information for license holders is considered confidential or exempt, therefore will not be provided. This includes social security numbers, birthdates, home addresses, and personal phone numbers.
