To register with the North Dakota State Board of Chiropractic Examiners please complete the following:

Print and fill out our Application for Examination

Please mail your completed "Application for Examination" and the following documents to the Executive Director:

  1. Three letters of reference
  2. Photocopy of birth certificate
  3. Personal resume since high school graduation
  4. Photocopy of Chiropractic college diploma
  5. Chiropractic college transcript
  6. National Board transcript
  7. Enclosed guaranteed US funds in the amount of $200.00.
  8. Passport size photo and approximate date taken.
